Commissioner Howard Joins Camden Coalition of Health Care Providers to Announce HIT Initiative to Improve the Care of Chronic Disease Patients


 

Camden - On Tuesday, September 22 at 11 a.m., Health and Senior Services Commissioner Heather Howard will join Dr. Jeffrey Brenner, medical director of the Camden Coalition of Healthcare Providers, to announce the creation of a groundbreaking electronic health information exchange that links private physicians, hospital emergency rooms and community health centers as part of a citywide collaborative to improve the care of patients with chronic diseases like diabetes.

 

Dr. Jeff Kleeman, a family practice physician in Fairview, will give an overview of how electronic health records transformed care in his office. Two patients will talk about the current system of diabetic care, and a hospital representative will give an overview of the benefits of Health Information Technology. HIT promotes both efficiency and improved quality of care and is central to national health care reform.

 

The event will take place at the Fairview Village Community Center, 2961 Yorkship Square, Camden.
