WHEN TO USE IT

 

Adolescents age 18 to 21 (i.e., young adults) in out-of-home placement complete the form annually.

 

The form documents whether the adolescent gives his or her approval for the CP&P Worker to view, and review the adolescent's annual credit reports with the adolescent, and whether or not the adolescent gives his or her consent for the CP&P Worker to assist with resolving any issues associated with inaccurate information, fraud, or identity theft.

HOW TO USE IT

 

The CP&P Worker prints out the form and then meets with the adolescent to review it together.  The adolescent then completes and signs the form.

 

TIPS FOR COMPLETING THE FORM

 

·         Print or type in the name of the adolescent and his or her CP&P Worker.

 

·         The adolescent checks the applicable check box to indicate if they give their consent or not.

 

·         The adolescent signs and dates the form.
