How to Submit a Waiver Request

Electronic submissions are preferred.   

Preparation:

• Gather the necessary information in advance to complete the application.  The online form will “time-out” after 10 minutes of inactivity and any progress up to that point will be lost. 
  
  
• If you need to submit supplemental information, follow the instructions on the How to Submit Supplemental Information with your Waiver Request
