How to Submit Supplemental Information with your Waiver Request

To submit supplemental information:

• Provide the documents as an attachment to the online waiver request form.  (Instructions are provided within the form.)
  
  
• If you cannot submit an attachment electronically or if you are submitting confidential/proprietary information, create a cover letter within the online form and mail the hard copy information.  (Instructions are provided within the form.)
