Search for Waiver Requests and Decisions

You can track any specific waiver request, search for waiver requests by certain categories, and generate reports associated with your search.

Click on the “Search for Waiver Requests and Decisions” button on the top of the Waiver Web Page or click the link below. 

Use the “Reports by Category” button at the top of the page to find the Waiver Request Reports.  Here you will find a list of reports, choose the one that is appropriate and follow the prompts on the screen to generate your report results.
