Pursuant to N.J.S.A. 17:12B‑171 each state chartered association is required to file with the Commissioner of Banking an Annual Report as of the close of its fiscal year and additional reports as required. Included below are links to the report forms which are to be used for compliance with Section 171. These reports request structural information for all associations as of December 31, 2012. Unless otherwise requested, this will be the only filing for structural information in 2013.
The structural information requests the following two reports:
- The first report is to be completed by listing the entire current board membership of the Association. Section 171 requires that this report disclose the amount of compensation paid to each director. Please indicate in the last column a letter which corresponds to the range of compensation paid to each director. Compensation includes salaries, director fees, retainers and any bonus. - The second report has been designed to identify officers along functional lines (Chairman of the Board, President, Chief Executive Officer, Compliance Officer, etc.) within the Association. It is realized that in some instances an individual's name may be listed on the form more than one time. Section 171 requires that the annual report disclose the compensation paid to officers, attorneys, and other employees. Please indicate in the last column a letter which corresponds to the range of compensation paid to each officer, attorney and other employee. Compensation includes salaries, director fees, retainers and any bonus. If, during the ensuring year, there is any change to the membership of the board of the Association or if there is any change to the officers of the Association identified on this Annual Report, please notify the Chief Examiner of Applications and Corporate Filings of the Division of Banking Office of Depositories in writing with a description of the change as soon as possible. In addition to the two reports described above, the following supplemental reports are to be submitted as part of the annual report filing.
The due date for filing these reports is February 15, 2013, which includes the 10-day extension allowed by N.J.A.C. 3:4-3.3(a). Please e-mail the completed annual report forms to the Division at Dobi.email@example.com. Alternatively, you may return the completed forms by mail to:

Under the provisions of the New Jersey Administrative Code 3:7‑5 et seq., every person who is required to file Statement of Interest forms (SI-1 and SI-2) with your association shall complete and file both of these forms within 30 days after the annual meeting. In the event an individual already has a statement on file, and there is no change or only a minor change, the annual review may be accomplished by signing and re-dating the certificate already on file on the second page of the form. As an alternative, the individual may issue a certification similar to the one contained on the second page of the form making reference to the statement on file. All statements shall be kept on file at the bank in accordance with N.J.A.C. 3:7‑5.4. For your convenience you may use these links to the forms: SI-1 and SI-2.
