ELL Intake Procedures

Registration Forms: The registration form, if possible, should be both in English and student's first language. A parent volunteer who speaks the family's first language can also assist in completing forms. The form should ask for the following information:

• Name
• Address
• Home telephone number
• Contact information for parent[s] in case of emergency or illness [to be placed both on the registration form and in the health office]
• An additional contact person, in case the parent cannot be reached [to be placed both on the registration form and in the health office]
• Country and date of birth [birth certificate or passport] to help determine age and grade placement
• Language[s] spoken by the child
