An Asthma Treatment Plan (ATP) is a written plan that tells people with asthma, or their caregivers, how to manage asthma symptoms. The plan explains what to do every day to prevent symptoms from happening and what to do if symptoms start or worsen. When used properly, the plan can help people control their asthma. The New Jersey Asthma Awareness and Education Program recommends that all asthma patients have an ATP. Visit the Pediatric/Adult Asthma Coalition of New Jersey (PACNJ) at www.pacnj.org to download a free ATP for you or your child. This new plan is available in English or Spanish and meets New Jersey Law N.J.S.A.18A:40-12.8 for an Asthma Action Plan. Asking your or your child’s healthcare provider to personalize this plan will help you take action on asthma.
This
brochure (Asthma Needs PAC Action) will help you learn about asthma and how to control asthma.
