J-1 Visa Waiver
Application Case Number
To start the J-1 visa waiver application process, an applicant needs to communicate with both federal and state agencies. Please carefully read the following instructions before applying for a case number.
The New Jersey Department of Health (DOH) recommends that before submitting a Data Sheet and filing fee, applicants contact Linda D. Anderson at (609) 292-1495 or email:
to determine that the employer/facility and the applicant/physician both meet the minimum requirements to qualify for a waiver. The determination that the employer and the applicant meet the minimum requirements does not guarantee that the full application will be approved by DOH.
US Department of State Requirements:
Prior to applying for a J-1 visa waiver and applicant must send a completed Data Sheet, two self-addressed, stamped, legal s-size envelopes (S.A.S.E.), and a $230.00 filing fee to the United States Department of State and receive a case number. The case number must appear on all documents filed with the application.
