Program Description
New Jersey is committed to assisting families experiencing complex, long-term medical and developmental disabilities by helping them navigate systems of care. The FCCS program promotes access to care through early identification, referral to community-based, culturally competent services, and follow-up for Children and Youth with Special Health Care Needs (CYSHCN) from birth to early adulthood.
Services and Supports
Services and supports provided through FCCS include:
- Special Child Health Services Case Management Units (SCHS CMU): Resource and referral services for families of CYSHCN ages birth through 21 years
- Fee-For-Service Program: Financial Assistance for Children and Youth with Special Health Care Needs ages birth through 21 years
- Ryan White Part D, Family Centered HIV Care Network (RWPD): Infants birth to 2 years exposed to or with HIV, Children and Youth ages 2 through 24 years with HIV, low-income women (25 years and older) with HIV
- Specialized Pediatric Services Program: Pediatric specialty and sub-specialty services to families with CYSHCN
