The Chronic Disease Self-Management Program (CDSMP)

The Office of Minority and Multicultural Health is proud to promote the Take Control of Your Health Program. This is a fun and practical course that can help people with chronic conditions overcome daily challenges, and maintain an active, fulfilling life. Participants learn about healthier ways to live, gain confidence and motivation to manage their health, and feel more positive about their lives. They are provided with information, strategies and techniques, so they will have the tools they need to help themselves.
Adults of all ages with chronic conditions such as arthritis, diabetes, heart disease, asthma, fibromyalgia, hypertension, depression or any other ongoing or long-term health conditions as well as family members, friends and caregivers, will benefit from the information and better understand living with a chronic condition.
Course participants learn to: manage symptoms; get started with healthy eating and exercise; communicate effectively with their doctor and health care team; manage their fear, anger and frustration; make daily tasks easier; and get more out of life.
Research confirms participants increase healthful behaviors, believe their health has improved, are less limited in their daily activities, are less bothered by their illness, and have greater confidence in their ability to manage their condition.
The program is delivered in various community settings such as recreation centers, libraries, community health centers and seniors’ activity centers. The course is offered at no or low cost to participants and they may receive a copy of the excellent reference book Living a Healthy Life with Chronic Conditions.
The Office of Minority and Multicultural Health collaborates with the Division of Aging and Community Services (DoAS) in the NJ Department of Human Services.
