Non-Expedited Mail Order of Genealogical Records

You can mail your completed "Genealogical Vital Record Application" (REG-28) to the Office of Vital Statistics and Registry or to the Local Vital Records Office in the city or town where the vital event took place. The records you requested can only be sent to the address listed on your identification documents.

    • For a Local Vital Records Office, call the local office for information on processing times and fees.

    • For the Office of Vital Statistics and Registry:

      • You may also send your application via express courier to the Office of Vital Statistics and Registry; however, your document will be returned via regular mail unless you include a pre-paid return express courier envelope.

Remember to include with your completed application form, Acceptable Forms of Identification and the appropriate fees for a copy of a vital record. If you are requesting a certified copy you will need to include proof of relationship as well.
