The New Jersey Division of Developmental Disabilities' Supports Program is a Medicaid program. All Medicaid-eligible individuals who have been determined to meet the functional criteria to receive services funded through the Division will access those services through the Supports Program*.
*Medicaid does not allow any individual to be enrolled on more than one Medicaid program. Therefore, Medicaid-eligible individuals enrolled on the Community Care Waiver (CCW) will not access disability-related services through the Supports Program and will only access those services through the CCW.

Provider Qualification Process Temporarily on Hold Please be advised that effective October 20, 2014, the qualification of new service providers will be temporarily on hold. Any applications currently in process or those in transit and received by October 17, 2014 for open services at this time will continue to be processed. The Day Habilitation qualification process continues to be on hold.
This change is necessary due to the development of the Medicaid provider application process as part of the Division’s system reform efforts. The Medicaid provider application will remain open for Support Coordination services only. Please check this page regularly for updates regarding the Medicaid application process.
