All Medicaid-eligible individuals who also are eligible for DDD functional services will be able to access the Supports Program, unless they are enrolled on the Community Care Waiver. The Supports Program is a Medicaid program, which requires individuals to maintain their Medicaid eligibility in order to continue to participate.
Updates on the Supports Program
Updates as of April 26, 2013:
(1.) On Monday, April 15, 2013, the Division of Developmental Disabilities (Division) held a webinar for providers interested in providing support coordination entitled, “Initial Phase of Support Coordination for the DDD Supports Program.” The PowerPoint presentation for this webinar can be found below under Presentations: PowerPoints, YouTube. We have also created a document of questions that came up as a result of the webinar and the webinar can be found under Provider Information.
(2.) The Division is also very excited to announce that the Provider Application for Support Coordination is now up on our website. You can access the application from our Supports Program Provider portal located here: http://www.state.nj.us/humanservices/ddd/programs/sppp.html.
(3.) We have also placed the Conflict Free Policy for the Supports Program listed under Provider Information.
Types of Services Provided
Employment/Day Supports:
DDD will provide employment services and other day supports to supplement employment when an individual’s educational entitlement ends.
Individual and Family Supports:
DDD will provide self-directed individual and family suppor t s such as adapt ive technology, behavioral supports, env i ronment a l and vehicle modifications, respite, etc. via a self-directed model based upon assessed need. Individuals will choose from approved providers using established budgets.
