Every individual receiving services from the Division has a Case Manager or Support Coordinator to manage his/her care and service plan. Care Management is an administrative service that is covered by the Division and does not come out of the individual's annual budget.

Case Management

Case management services help individuals learn about and gain access to services that can help address their needs. These include, but are not limited to, Medicaid waiver and Medicaid State Plan services as well as natural supports offered within the local community and appropriate services offered through other state agencies.

Transition Case Managers provide direct assistance and coordination to any of the residents of the state developmental centers who are preparing to move out of a developmental center and into a community residential program.

 
 
Support Coordination

Individuals who have enrolled or transitioned into fee-for-service select or are auto-assigned to a Medicaid/DDD-approved Support Coordination (SC) Agency. The SC Agency then assigns a Support Coordinator. Individuals can change Support Coordination Agencies upon request.

With the individual and his/her family, the Support Coordinator develops the Person Centered Planning Tool; assists the individual in identifying service needs and researching service providers; and writes the Individualized Service Plan (ISP), which authorizes services and service providers. 

Support Coordinators assist individuals with accessing services from:

  • natural supports offered within the local community
  • appropriate services offered through other state agencies
  • DDD-funded programs and services

Support Coordinators conduct monthly monitoring to:

  • ensure the individual is receiving the services authorized in the Individualized Service Plan
  • ensure the services continue to meet the individual’s needs
  • provide follow up as needed
  • check in on the individual’s health and safety 

Monitoring is conducted with:

  • monthly phone contacts
  • quarterly face-to-face visits
  • annual home visits
