Division-funded services for adults with intellectual and developmental disabilities are not an entitlement and are dependent on resource availability.
DDD Provider Database
The Division is in the process of transitioning from a contract-based model of provider reimbursement to a Medicaid-based, fee-for-service model. This model, already successfully utilized in a number of states, allows for increased flexibility, increased choice, and an expansion of services available. The transition to a Medicaid-based, fee-for-service system is anticipated to occur July 1, 2015. Once the new system is fully implemented, adults seeking Division-funded services must be enrolled on one of two Medicaid waiver programs: the Supports Program or the Community Care Waiver (CCW). Enrollment on these waiver programs is based on assessed level of need, and the Division currently maintains a Waiting List for individuals requesting services through the Community Care Waiver.
We are pleased to offer you the DDD Provider Database. This searchable website allows you to browse hundreds of health and human service agencies or individuals that have completed the approval process through DDD to provide fee-for-service supports to participants self-directing their services based upon their plans of care. The DDD Provider Database offers the ability to search in multiple ways, such as by county, agency name, and service type.
