Eligibility and Application Process
To be eligible for a free device through the DDHH Equipment Distribution Program, New Jersey residents with hearing loss must be from a household with a combined income of less than $45,000 and must have the application signed by a licensed audiologist or physician, verifying applicant's hearing loss. The form must be clearly printed and signed by the applicant.
Review the catalogue of available devices. Obtain an application for a specific device, print it and fill it out clearly with the required signatures. Mail or fax the application to the DDHH offices at
