The Department of Human Services' Division of Mental Health and Addiction Services (DMHAS) is working in collaboration with the Division of Medical Assistance and Health Services (Medicaid) to develop the Behavioral Health Home.  As part of the process, DMHAS and Medicaid must submit a State Plan Amendment to the Federal Centers for Medicaid and Medicare (CMS).  The materials below are elements of New Jersey's submission to CMS, including program design and the services.
