Employee Awards Instructions
This form is in Microsoft Word and can be filled in on the computer. There is no need to use a typewriter or hand write the form anymore. Begin by typing your name and location in the first available box (Box 2) then press the Tab key to move to each subsequent box.
Follow the instructions below for filling in the numbered boxes of the form:
What To Do with Each Box:
Box 2 - Type your name and location.
Box 3 - Click on the check box preceding the words Employee of the Year, Employee of the Trimester, or Team of the Trimester. (When you click on Employee of the Trimester or Team of the Trimester, the Tab key will take you to the beginning of Trimester date. Type the date that begins the Trimester of the nomination period, then press the Tab key again and type the date that ends the Trimester of the nomination period.)
Box 4 - Type the name(s) of the person(s) you are nominating, press the Tab key and type his/her/their title(s), then press the Tab key and type his/her/their location.
Box 5a - Type a short paragraph about the work habits of the person or group.
Box 5b - Type a short paragraph about the work quality and/or quantity of the person or group that makes him/her/them worthy of special recognition.
Box 5c - Type a short paragraph about the accomplishments of the person or group that makes him/her/them worthy of special recognition.
Box 6 - Type your position title. (the current date will appear automatically; you may print the form to sign it)
