REGISTRY POSTING of ADMISSIONS and DEATHS

POLICY STATEMENT.

The Division of Veterans Healthcare Services (DVHS) requires that each of the New Jersey Veterans Memorial Homes (VMH) maintain a “Registry” of accessible information related to all Admissions and Deaths within the VMH.

PURPOSE.

This policy and procedure serves to ensure that each of the New Jersey Veterans Memorial Homes maintains a “Registry” of accessible and retrievable information related to all Admissions and Deaths within the VMH.

PROCEDURE.

The Medical Records Department shall maintain a Registry of accessible information.

  1. The information to be recorded in the Registry is obtained from the following resources:
    1. Admitting Record(s)
    2. Transfer Form(s)
    3. Death Certificate
    4. Medical Record
  2. Upon Admission, the registry posting is to contain:
    1. Resident's name
    2. Resident's VMH ID number
    3. Resident’s Date of Admission
    4. Resident’s Date of Birth
    5. Facility from which he/she was admitted
  3. Upon Death, the registry posting is to contain:
    1. Resident’s Name
    2. Date of Resident’s Death
    3. Resident’s VMH ID Number
    4. Resident’s Unit
    5. Location of Death
