the Department of Transportation.

Contractor Prequalification Requirements
Firms must complete and submit a Revised Contractor's
Financial and Equipment Statement Form DC-74A (pdf
219k) (dot 860k) to Construction Services and include a Certified
Audited or Reviewed Financial Statement, along with a
$100.00 check made
payable to "Cashier, NJDOT". Mail all to the
above address.
- In order to view and print Form DC-74A, you must
have Adobe Acrobat Reader. This software is available
at no charge at the State's Adobe
Reader access page.
- The form may be requested by phone at (609) 530-2103
or by fax at (609) 530-2238.
- Rules and Regulations relevant to contractor prequalification
are included with the form.
