A New and improved Two-Year Chapter 51/Executive Order 117 Vendor Certification and Disclosure of Political Contributions form is now available on the Treasury website.
The form can be found online by going to the following web link and scrolling to “VENDOR FORMS REQUIRED FOR CONTRACT AWARD.”
Please note that the Information and Instructions form (I&I) and the C.51/EO117 Certification form (C.51) are now in the same PDF link with the Instructions first.
One of the key differences in the new form is it is no longer necessary to submit an Ownership Disclosure form along with the C.51 form which was needed with the old version. The ownership information required for Chapter 51 purposes is now incorporated into the new form.
Another key change is the time period of contribution disclosures has been changed to include up to 5 ½ years (for certain contributions) whereas the old version requested the preceding 18 months (for certain contributions). The certification options have been increased from a choice of three (3) to a choice of four (4). As was the case with the old version, by checking BOX A the authorized representative completing the form is certifying on behalf of the business entity and all individuals and/or entities whose contributions are attributable to the business entity.
- The rating cycle will now end December 31. The new rating cycle will be a calendar year from January through December.
- Rating reminders will be sent in November so PM’s can start rating in December. Ratings will be due at the end of January.
- Concept Development task orders will no longer be rated under the Planning discipline. All CPM general engineering term agreements and associated task orders will be rated under Design.
- There is no Overall Quality rating for task orders executed to assist in-house design forces. Overall quality is a project specific agreement rating.

Please redact this information from certificates and/or resumes before including them in a TP.

Ensure that proposed professional services firms
(sub-consultants) are cost basis approved prior to the closing date of the
solicitation or modification proposal by checking the website. NJDOT Professional Services - Cost Basis
These firms must be identified in
the sub-consultant section of the proposal and not Direct Expense. Both Federally
and State funded contracts must utilize firms that are cost basis
CONSTRUCTION INSPECTION TECHNICAL PROPOSALS
Project Manager requirements have changed. All Project Managers must be an employee of the Prime Consultant. This change can be found in the instructions on Page 1, Part 2 Section 1A of the definitions.

NEW FOR CYCLE 22
CONSTRUCTION INSPECTION RATINGS:
If the total rating for an agreement is less
than 3.80, the Regional Consultant Coordinator (RCC) will e-mail a copy of the
ratings to the Consultant. The Consultant will be afforded the opportunity to
appeal the rating with the Rater and Rater's Supervisor. As a result
Professional Services will not be distributing the pdf rating files for all
Construction Inspection projects in the Cycle 22 rating period. If any
agreement ratings are below 3.0, firms may ask for a debriefing to discuss the
