REDUCE YOUR HEALTH CARE COSTS THROUGH A
DIRECT PRIMARY CARE MEDICAL HOME (DPCMH)
FOR SHBP/SEHPB MEMBERS

Last year, the State Health Benefits Program (SHBP) and School Employees’ Health Benefits Program (SEHBP) introduced the Direct Primary Care Medical Home (DPCMH) initiative focused on improving the physician-patient partnership and reducing health care costs!  This innovative initiative offers a no copayment, no deductible benefit that works with your current health plan; no need for you to change plans.  DPCMH services are provided by R-Health and Paladina Health.

The concept of a DPCMH is to provide primary care services in very different ways from a typical primary care practice.  Services include preventive, urgent, and sick care, health and wellness coaching, chronic disease management, and care coordination.  Patients get direct access to their physician with same-day or next-day appointments, evening and weekend hours, reduced waiting-room time, and unhurried consultations with their physician.  This is all provided with no co-payment or deductible.
