If a member wishes to participate
in this plan, a SACT Enrollment
Request Form must be completed.
The employer, must certify the application
and send it to the Division of Pensions and Benefits.
Enrollment applications are processed
quarterly and must be received by the Division of Pensions and
Benefits, SACT Section, not less than 30 days before the beginning
of any calendar quarter.
Once an enrollment has been processed,
the employer will receive copies of a certification of payroll
deductions. The enrollment will become effective at the start
of the next calendar quarter.
Each enrolling participant is also
asked to nominate primary and contingent beneficiaries.
Any time a member has a change in
family status (e.g., marriage, divorce, death, etc.), the beneficiary
designation should be updated by submitting a completed Beneficiary
Designation Form.
