PPO 10 and HMO plans have a $10 copayment for primary physician visits and for specialist visits.
PPO
15 plans have a $15 copayment for primary physician visits and for specialist visits.
1525 plans have a $15 copayment for primary physician visits and a $25 copayment for specialist visits.
2030 plans have a $20 copayment for primary physician visits and a $30 copayment for specialist visits.
HD4000 plans have a $4,000 in-network deductible.
Medicare eligible retirees cannot enroll in Aetna Freedom2030, Aetna HMO 2030,
or any of the High Deductible Health Plans.
