FOR PLAN YEAR 2014

AETNA PLANS

PPO Plans

    HMO Plans
HORIZON PLANS

HMO Plans

PPO 10 plans have $10 copayment for primary physician visits and for specialist visits.
PPO 15 and HMO plans have $15 copayment for primary physician visits and for specialist visits.
1525 plans have $15 copayment for primary physician visits and for $25 copayment for specialist visits.
2030 plans have $20 copayment for primary physician visits and for $30 copayment for specialist visits.
HD4000 plans have a $4,000 in-network deductible.
Medicare eligible retirees cannot enroll in Aetna Freedom2030, Aetna HMO 2030, or any of the High Deductible Health Plans.

Medicare Eligible Retirees may choose any NJ DIRECT or Horizon HMO plan
or one of the following Aetna Medicare Advantage Plans:

Aetna Medicare Advantage PPO ESA 10
Aetna Medicare Advantage PPO ESA 15
Aetna Medicare Advantage HMO
Aetna Medicare Advantage HMO 1525
