What Do I Do if I Don't Have a Payment Voucher?

Print a Payment Voucher:

NJ-1040-V (residents)
NJ-1040-NR-V (nonresidents)

If you cannot print a payment voucher:

If you do not want to use e-check or pay by credit card, mail your check with a handwritten note stating your name, address, social security number, and the tax year and tax type ("2012 NJ Gross Income Tax") for which the payment is being made. Make the check or money order payable to State of New Jersey – TGI. Send your payment for the balance due along with the handwritten note in the same envelope as your tax return.
