WHEN TO USE IT
The affidavit, CP&P Form 14-86f, is used when a surrendering parent desires to identify the individual(s) that he or she wants to adopt his or her child that who is the subject of the Surrender of Custody and Consent to Adoption, CP&P Form 14-86.
HOW TO USE IT
• Access it through the Create > Casework > Court/Legal > Options Drop-down.
• Only complete this template outside of the NJ Spirit application as part of a contingency plan when the application is unavailable. However, in doing so, you are still required to create the form in NJ Spirit when the application becomes available.
• Print the form for approval and signatures, as explained.
TIPS FOR COMPLETING THE FORM
CP&P Form 14-86f is now a template to be completed
in NJ SPIRIT. Prepare the form in quadruplicate. The Worker fully explains the
meaning of the surrendering parent's signature on the form.
The Worker reads the affidavit aloud and the surrendering parent follows along from another copy.
The Worker enters the required specified information utilizing information supplied by the surrendering parent into the text form fields. Each text form field includes specific instruction required for each field. The surrendering parent signs the affidavit in the presence of two CP&P representatives, the Local Office Manager and the Supervisor, or, when this is not possible, two Supervisors. The Worker ensures this form is witnessed, stamped, and signed by a notary after its completion.
