PURPOSE AND USE
The form is used to obtain the consent of a parent or guardian for a CP&P client to be photographed, interviewed or identified by the press, film or television. Unavailability of the parent or guardian is construed as denial of consent to photograph, interview or identify the child.
INSTRUCTIONS FOR COMPLETING THE FORM
Print the form from the manual. The child's parent or guardian indicates consent or no consent by circling the desired category as listed on the consent form.
The parent signs and dates the form and the Worker signs as the witness and indicates the name of the Local Office.
