CP&P Form 5-8, Resource Family Rate Assessment

WHEN TO USE IT

 

Use this form to determine the number of hours a Resource Family Provider will spend each week providing the services, performing the tasks, or conducting activities that are above what is expected in providing care for a child of that age.

The number of hours required for each of the services, tasks, or activities is jointly determined by the Worker, Resource Family Support Worker and Nurse in consultation with the Resource Family Provider, based on the presenting needs of the child.

Note: Under Transportation, medical visits, "Required/Routine" is equivalent to a well visit or a routine annual physical exam. "Additional Specialty-Quarterly, Monthly, or Weekly" visits are medical visits that are conducted and monitored by specialists.

The rate assessment is completed:

         Within 72 hours of a child's placement into a Resource Family placement; or

         Before the Kinship Legal Guardianship (KLG) or Adoption Subsidy is processed, and sent to the Office of Adoption Operations.

 

For a child in a Resource Family placement, a reassessment is conducted:

 

         At least every three months;

         Whenever a child moves from one home to another;

         When the child's needs or circumstances change; or

         When the child's acuity is reassessed by the Nurse.

 

An acuity level of 5 will receive an increased rate of $250.

 

HOW TO USE IT

 

         The Worker, Resource Family Support Worker, and Nurse, in consultation with the Resource Family Provider jointly conduct the assessment.

         Circle a, b, c, or d for each of the categories listed.

         For selections of b, c, or d, a written justification is required.

         The Worker enters data from the CP&P Form 5-8 into the Rate Setting Window in NJ SPIRIT and obtains a level of care rate, which prefills the CP&P Form 5-8a, Resource Family Rate Agreement.
