The annual report must be completed, then signed and notarized electronically.
*Update on Notarization - Recent legislation passed (P.L. 2020, c.26) allows notaries and authorized officials to execute remote notarial acts during the public
health emergency and state of emergency declared by the Governor in Executive Order 103 of 2020. For questions regarding notarization of the Annual Report,
please contact Guy Fasanella at email@example.com.
Upon completion, the report must then be sent via e-mail to firstname.lastname@example.org.
You may send a paper check to the Department of Treasury at the address provided below; OR make a payment via credit card using DEP Online
(https://www.nj.gov/dep/online under the “Pay a Paper Invoice” link). Please note that there is a
convenience charge for utilizing the online payment system (2.0% of total fee + $.50).
