Attention Registered Back Office/Service Facilities in New Jersey |
Your registration is set to expire on August 31, 2016 and needs to be renewed for an additional 2-year period until August 31, 2018. |

Here’s what you need to do to renew your registration: |
1. |
Complete the application for renewal for each back office/service facility you need to register. |
2. |
Provide a check for $500 for each facility to be registered. The check should be made payable to: “Treasurer, State of New Jersey” |
3. |
Mail the application(s) and check to

Department of Banking and Insurance

PO Box 040 (for regular mail)
or
20 West State St. (for overnight delivery)
