
Requests for corrections may be filed with the Office of Vital Statistics and Registry using the REG-15 (Application to Amend a Vital Record) or the Local Registrar in the municipality where the event occurred, or in the case of marriage or civil union license, in the municipality where the license was issued.
Requests for corrections must be supported by verifiable documentation. Requirements for corrections vary based on the type of correction being filed. Expedited Online Service
Please submit your amendment request when you place your order online by clicking the ‘Order a Vital Record’ tab and choose ‘Online Requests’ or click here. You must upload your REG-15 (Application to Amend a Vital Record) or complete a TYPED letter explaining the field changes you are requesting to be corrected. You must also upload verifiable documentation to support the correction requested as well as your proof of eligibility to obtain a certified copy of the record. Please note that adoption and legal name change requests are currently excluded from this online service.
