The Division of Developmental Disabilities (DDD) maintains the Community Care Program (formerly Community Care Waiver) Waiting List, which is a waiting list for determination of eligibility for enrollment in the Community Care Program (CCP). Waiting List status letters are mailed annually to all individuals in the priority category of the waiting list.

• Criteria for Addition to the CCP Waiting List
  An individual may request addition to the CCP Waiting List if they:
  • meet the Division’s functional criteria in accordance with N.J.A.C. 10:46
  • are 18 years or older
  • are Medicaid eligible

  OR if they:
  

  • were found eligible to receive DDD services prior to January 22, 2013, and were born prior to January 1, 1997, and are Medicaid eligible.

  Individuals who meet the above criteria and are currently served through the Department of Children and Families Children's System of Care (CSOC) can request addition to the CCP Waiting List. Please note that DDD services are only available to individuals 21 years of age and older who meet the DDD functional criteria and are Medicaid eligible.
  
  To be enrolled in the Community Care Program (formerly Community Care Waiver), an individual also must meet the specified level of care for CCP at the time they are reached on the CCP Waiting List.

  To request to be added to the Community Care Program Waiting List, complete the Community Care Program Waiting List Request Form.

  Submit the form, together with any supporting documentation, by email to: DDD.CCPWaitListRequests@dhs.nj.gov, or by mail to:
