Courtney knowingly concealed relevant information on his application for automobile insurance with Progressive Insurance Company on August 28, 2017. Specifically, Mr. Courtney indicated he resided and garaged his vehicle in Montclair, NJ, when in fact he resided and garaged his vehicle in East Orange, NJ.

Respondent, Patricia Montgomery, knowingly provided false and misleading information to United Services Automobile Association (USAA), when she filed a homeowner's property damage claim in reference to water damage.  Specifically, on May 25, 2017 and May 31, 2017, Patricia Montgomery submitted two falsified receipts for repairs to her mink jacket (May 31, 2017) and a receipt for a computer diagnostic (May 25, 2017) in support of the claim.

Respondent, Asha Godfrey, knowingly provided false and misleading information on an auto endorsement dated December 4, 2017.  Ms. Godfrey added a 2009 Acura TL that was not garaged at the policy address.  Additionally, Ms. Godfrey reported that she was the driver in a motor vehicle accident that occurred on January 3, 2018, when in fact she was not.

Respondent, Coleen Kelly, knowingly provided false and misleading information when reporting a homeowners loss to Universal North America Insurance Company dated January 9, 2018.  Specifically, Ms. Kelly knowingly submitted original plumbing repair invoices for reimbursement, instead of the renegotiated invoices, the difference totaling $1,315.

Respondent, Samantha Martina, did knowingly make a material misrepresentation to USAA Insurance Company on July 16, 2017, when she reported her vehicle stolen, when, in fact, it was not and she was involved in an at-fault accident.

Respondent, Jenna R. Boccia, did knowingly provide false and misleading information to CURE Auto Insurance Company by submitting a claim for rental reimbursement and misrepresenting the date of the loss to be August 8, 2017, after she added rental reimbursement to her automobile policy knowing she did not have rental reimbursement coverage at the time of the loss on August 7, 2017.

Luis Lincango-Penaherr, Bound Brook, NJ

Respondent, Luis Lincango-Penaherr, did knowing provide materially false information on an application for a Supplemental Sickness Insurance Policy with Combined Insurance Company of America dated May 2, 2017, by answering "No" to a qualification question, knowing that he had been treated on April 28, 2017 for a listed condition.

Curtis Johnson, Trenton, NJ

Respondent, Curtis Johnson, did knowingly make a materially false statement to Progressive Auto Insurance Company when he applied for automobile insurance on May 1, 2018, by providing the name of another individual.  Respondent, Curtis Johnson, also made a material misrepresentation when he submitted damage photos from March 25, 2018, during a claim for the loss that occurred in June 2018.
