You may express your wish to donate your organs and tissues for transplant by designating the words "Organ Donor" to appear on your New Jersey driver's license or non-driver identification (ID) card. This designation is for individuals 18 or older.
Register for organ donor designation
Organ donor registration is available online or in-person at a motor vehicle agency. When registering at an agency, you must present the required 6 Points of ID well as verification of your address. Remember that you must confirm your decision each time you renew your driver's license or non-driver ID.
Effective October 1, 2017, if you wish to solely add the organ donor designation to your existing driver license or non-driver identification card, you may do so at no fee, upon surrendering your current driver license or non-driver identification card.

Change an organ donor designation
To change your organ donor status on your driver's license or non-driver ID card, visit a motor vehicle agency. You must present the required 6 Points of ID as well as verification of your address. To submit your change request by mail, download a Change of Status form, complete and mail to:
